A specific combination of environmental elements in the sci-fi action game, Stellar Blade, that could be construed as a reference to a racial slur, will be eliminated in a day one patch, according to Sony's recent announcement. Sony clarified that the offensive reference was not deliberate.

Controversial Graffiti in Stellar Blade
The word “hard” in pink spray paint is seen in graffiti throughout Stellar Blade. According to reports, it appears next to a neon sign for “R shop,” creating an unintended offensive phrase.
“The juxtaposition of two graphics in Stellar Blade resulted in an unintentional offensive phrase,” Sony stated. The company confirmed that the offensive graffiti will be replaced in the Day 1 patch.
Removal of Offensive Imagery
The offensive reference seems to have been already removed from the game. A new blue graffiti reading “crime” has been substituted. Sony has not yet clarified if this change will be implemented in later disc printings of the game, or only be patched out via download.
It’s not uncommon for offensive imagery and references to find their way into finished games. In the past, several gaming companies have had to remove offensive elements from their games post-release.
